Step-by-Step Procedure

Hairline Planning and Design

The first step was careful hairline design. For Rocky, the goal was to restore a natural, age-appropriate hairline that blended seamlessly with his existing hair. Both the frontal area and the crown (where mild thinning was visible) were marked for implantation.

Donor Area Examination

Dermatoscopy confirmed that Rocky’s donor area had thick, healthy black hair suitable for extraction. This was an important factor in ensuring a good density and natural result.

Anesthesia

Local anesthesia was administered using a painless technique. Rocky reported only minimal discomfort, similar to a small ant bite, during the injection. After that, the procedure was completely comfortable.

Slit Making

Tiny incisions (slits) were created in the bald areas using a diamond sapphire blade. This stage is critical because the angle and direction of these slits determine how natural the transplanted hair will look. Special attention was given to match Rocky’s natural hair pattern.

Graft Extraction

A total of 3224 grafts were harvested from the donor area using a 0.8 mm titanium punch under magnification. To preserve their quality, the grafts were stored in a specialized graft preservation solution at a controlled temperature.

  • Graft Analysis:

    • 20% were single-root grafts.

    • 80% were multiple-root grafts (double and triple).

    • This distribution ensured both density and a natural frontal look.

Implantation

The harvested grafts were implanted into the prepared slits using the K.E.E.P. implanter, an advanced tool that allows precise placement of hair follicles. This ensured correct depth, angle, and spacing, giving Rocky’s new hairline a natural appearance.

Post-Procedure Care

After implantation, a dressing was applied, and Rocky was prescribed medications to prevent infection and aid healing. He was also assigned a follow-up doctor who guided him through each stage of recovery.

Recovery and Results

Every hair transplant follows a predictable recovery pattern, and Rocky’s case was no different.

  • First 3 Months – Shedding Phase:

The transplanted hairs went through a natural shedding cycle. About 20–25% of transplanted hair sheds each month, which is expected. At this stage, it may look like no transplant was done, but this is a temporary phase.

  • 3–6 Months – Growth Phase:

Rocky started noticing new growth from the transplanted roots. By the 6th month, about 70–80% of his hair had grown back, although lengths were uneven. During this period, he also underwent PRP sessions at the 2nd, 4th, and 6th months to boost growth.

  • 12 Months – Final Result:

After one year, Rocky achieved full results. His hairline looked natural, and the density was significantly improved. He was advised to continue with minoxidil, supportive multivitamins, and booster PRP sessions every 3 months to maintain both transplanted and existing hair.
