Your doctor had contacted you 5 days ago and despite the multiple requests, you still have not yet provided a consent form to your doctor.
This cannot be a one-sided discussion. If you choose to share your experiences in an open forum, you must do the right thing and allow your doctor to address your situation in the forum as well so that our community can learn all the facts pertaining to your situation.
