Is a second hair transplant worth it after a poor first result? Dr. Mehmet Demircioglu on Reddit

My original reply to a Reddit post about trying a second hair transplant:

Second hair transplant can be worthwile, but it depends on why first hair transplant failed. Without identifying the cause of poor result, don’t book another hair transplant. Also a disappointing outcome doesn’t automatically mean your scalp is unsuitable for hair transplantation.

During my career as a hair transplant surgeon, I’ve operated many patients who had a poor result in their first hair transplant, and most got good results with a second session.

There is also important and critical difference between grafts that never grew and grafts that grow but were later obscured by continued hair loss of surronding native hair. Photos taken before surgery and throughout first post operative year can help distinguish it.

Poor growth may result from improper graft handling, high transection rage in donor extraction process, inadequate donor quality, improper incisions such as too superficial or too deep in comparison to average graft length, or scalp related conditions. Each cause requires a different approach.

Consider whether your hair loss was stabilized before the first hair transplant and whether you used medical treatment after surgery, if so, were the frequency of medication dosages accurate and sufficiently effective. For instance, many patients use topical Minoxidil as a sufficient option, however, as a surgeon I consider topical can be just combined with oral Minoxidil, instead of using it alone.

Also if you used Finasteride were you consistent about it, what was the dosage? If you couldn’t see results from Finasteride, did you switch to Dutasteride? Sometimes, those adjustments in medical treatment may change the progress and overall results of hair transplant.

Was your first hair transplant performed by a skilled surgeon at a reputable clinic? Or a random local hair clinic or a lower profile hair mill clinic. When a patient contacts us for a second hair transplant after a poor result elsewhere, I value that detail a lot. If he got a poor result with a reputable clinic and surgeon, then likelihood of achieving a satisfactory outcome from a second hair transplant may be low, although there may be some exceptions.
