Consultation at Hairfree & Hairgrow Clinic, Surat

During consultation:

  • Detailed scalp and hair analysis was performed.

  • Trichoscopy evaluation was conducted

  • The hair transplant procedure was explained in detail.

  • Advantages, limitations, and expected outcomes were discussed.

  • Patient expectations were thoroughly addressed.

After a complete understanding of the procedure and realistic expectations, Vinod decided to proceed with hair transplantation.

Treatment Profile

  • Procedure Date: 26th August 2023

  • Technique Used: I-FUE (Implanter FUE Technique)

  • Total Grafts Extracted: 4,060 grafts

Post-Transplant Plan

  • 5 PRP sessions at 2-month intervals over 1 year

  • Multivitamin supplementation

  • DHT blocker (Finasteride)

  • Minoxidil 5% and 10% topical solutions

This comprehensive plan was advised to support transplanted graft survival and maintain existing native hair.

Step-by-Step Hair Transplant Procedure

1. Planning and Hairline Design

Hairline design was performed by Dr. Jinkal Kunjadiya according to Vinod’s facial structure, age, and baldness pattern.

Since the patient had Grade 5–6 baldness, two sessions were recommended. In the first session, the frontal region, mid-scalp, and upper crown were addressed. The lower crown was reserved for a second session if required.

Trichoscopy findings:

  • Donor area: Average density

  • Hair quality: Thin to average

  • Hair type: Mildly wavy to straight

  • Hair color: Light black

An anesthesia sensitivity test was performed and found negative.

2. Anesthesia

Local modified block anesthesia was administered using a vibration-assisted pain-minimizing technique to ensure patient comfort throughout the procedure.

3. Graft Extraction

Graft extraction was performed carefully using magnifying loupes.

  • A 0.8 mm sharp serrated sterile titanium punch was used.

  • Extraction time: Approximately 130–150 minutes

  • Total grafts extracted: 4,060

Graft Analysis

  • 20% single-rooted grafts

  • 80% double- and triple-rooted grafts

  • Average number of triple-rooted grafts present

  • Hair shafts were thin to average in thickness.

4. Graft Storage

Extracted grafts were preserved at 4°C in RL solution using a graft chiller machine to maintain optimal viability.

5. Slit Creation

Slit creation was performed by Dr. Jinkal Kunjadiya using sapphire metal blades:

  • 1.2 mm blade for hairline

  • 1.4 mm blade for remaining areas

Proper angle and direction were maintained to ensure a natural hairline and appropriate growth pattern.

6. Implantation

Implantation was performed using K.E.E.P implanters of various sizes suitable for 1, 2, and 3 follicular unit grafts.

  • Implantation time: Approximately 3 to 3.5 hours

After completion, dressing was applied, and detailed post-procedure instructions were provided. The patient was also given direct follow-up contact for any queries.

Post-Transplant Journey

Shedding Phase (0–3 Months)

Shedding began approximately 15 days post-procedure. During this phase:

  • 60–70% of implanted hair shafts shed

  • Roots remained secure in the scalp

  • Two PRP sessions were administered.

Donor area healing:

  • Complete healing within 1 month

  • Normal donor density appears by 2–3 months

The patient was reassured regarding temporary shedding.

Baby Hair Growth Phase (3–6 Months)

  • New baby hairs began appearing after 3 months.

  • Gradual filling of frontal and mid-scalp regions

  • Third PRP session administered

  • 10–20% visible improvement each month

  • Significant cosmetic improvement by the 5th month

Active Growth Phase (6–9 Months)

  • Rapid growth of transplanted hair

  • More than 90% visible density achieved

  • Patient is able to style hair comfortably

  • Booster PRP administered at 9 months

The patient’s confidence improved significantly during this stage.

Final Result (10 Months)

Full results were achieved at approximately 10 months post-transplant.

  • Natural hairline

  • Improved density in frontal and mid-scalp areas

  • Patient underwent multiple haircuts

  • Continued use of minoxidil is advised

  • PRP boosters are recommended as needed for maintenance

Vinod expressed high satisfaction with the results and regained social confidence.

Conclusion

Vinod Jatoliya’s case demonstrates that advanced I-FUE hair transplantation combined with proper medical management and PRP therapy can deliver natural and long-lasting results even in Grade 5–6 baldness cases.
